After you have your fingerprints taken by the live scan service provider, either submit a completed copy of RE 237 with your application or, if you choose to get your fingerprints taken at a later date, mail a completed copy of RE 237 to:

Keep in mind that your license will not be issued until a report from the Department of Justice is received stating that there is no criminal history, or disclosing the existence of criminal history information. We know you’re excited to start your new career, so we recommend taking your fingerprints early and submitting the combined application before you take your licensing exam!
